About The CAHP Coach and Services Offered. The CAHP Coach is a natural health consultant who utilizes certain complementary and alternative educational resources, tools, trainings, techniques, and modalities to help clients to learn about and address observable balances/imbalances in the body and to reach their natural wellness and lifestyle goals. The CAHP Coach is not licensed, certified, or registered by the state of Arizona as a health care professional. The CAHP Coach’s professional mission is to inform and educate each client about processing health information so that they feel empowered to make their own decisions concerning their own health, lifestyle, nutrition, spirituality, and wellness; and that of their family, dependents and pets.
Nature of The CAHP Coach’s Services. For purposes of this Agreement, “CAHP Services” are defined as non-medical and non-therapeutic holistic wellness coaching services that are offered for the purpose to assist the Client in their efforts to self-educate and care for themselves so that they might reach their wellness goals and achieve their highest wellness potential. The CAHP Services are based on a combination of their natural wellness knowledge, education, and experiences; they are not state licensed medical or health care services. The CAHP Services are provided in a private client-coach relationship and are never provided in any form of medical provider-patient relationship.
Scope of CAHP Services. The CAHP Coach loves coaching others to reach their personal, professional, and lifestyle goals. CAHP Services include complementary and alternative philosophies and/or modalities such as natural nutrition, blood work evaluation, naturopathy, herbology, aromatherapy, energy work, meditation, Muscle Response Testing, Biofeedback units, questionnaire assessments, various evaluation tools and techniques. The CAHP Coach makes no guarantees as to the efficacy or results of any CAHP Services
No Medical Treatment. The CAHP Services may be viewed as complementary to the Client’s other medical or therapeutic care, however, the Client acknowledges and understands that the CAHP Services do not, and will not ever, involve or include the diagnosing, treating, curing, or mitigating of any conditions, illnesses, diseases, and/or mental disorders that the Client is now experiencing or may experience in the future. Similarly, the CAHP Coach is not permitted to admit the Client into a hospital or to be on call for emergencies. To put it simply, the CAHP Services do not constitute any form of medical practice or medical analysis. CAHP Services are never to be accessed or used by the Client as a substitute for medical care, medical analysis, counseling, psychotherapy, mental health care, substance abuse treatment, or other professional advice by qualified medical professionals.
Coaching Session Procedure. CAHP Service sessions are by appointment or zoom calls through the program. Each CAHP Service session will take place at the time and place agreed upon by the Parties. The CAHP Coach will strive to be available and attentive to the Client’s schedule needs.
Scheduling Sessions. CAHP Services are designed to accommodate the diverse needs and schedules of all clients. The Client may schedule sessions by [booking through Practice Better, emailing the CAHP Coach, calling the CAHP Coach, using online scheduling application available at: www.medellahealing.com;. The Parties can arrange to meet in-person or digitally via Zoom, FaceTime, Skype, telephone.
Telehealth & Telemedicine Disclaimer. all CAHP Services that the Parties agree to engage digitally or by telephone are to be referred to as “Private Digital Coaching Sessions” between the Parties, and that all such Private Digital Coaching Sessions are never to be interpreted or referred to as being a telehealth or telemedicine service or meeting of any kind.
Refund Policy. Due to the private and sensitive nature of the CAHP Services, products, and experiences, the CAHP Coach’s general rule is that no refunds will be provided after delivery of any CAHP Services, products, and experiences.
As a matter of principle, training, and valid legal reasons, the CAHP Coach promises to not presume, nor attempt, to diagnose, treat, cure, heal, prevent, or mitigate any physical, emotional, mental or spiritual injuries, diseases, or conditions.
Client Satisfaction; No Guarantees. The CAHP Coach will use their best efforts to provide CAHP Services to Client and shall perform all services in a diligent, professional, collaborative, and satisfactory manner. Client acknowledges and recognizes that, due to the nature of coaching and the importance of self-responsibility, the CAHP Coach cannot guarantee a particular result or the outcome of any CAHP Service.
